- Registered Voter & Copy of Voter’s Card
- Complete Application
- Attendance at Planning Meeting and Regular Meeting
- Copy of Certificate of Indian Blood (CIB)
- Copy of Social Security Card
- Class Schedule (Current Semester)
- Class Grades (if previously awarded)
- Official Transcripts
- Letter of Admission
- Photo ID
- Student Information Consent Form
- Other Requirements might be required for first time applicants, please check with Administration.
